Body Composition Effects
The clearest direct data in bodybuilders comes from a double-blind placebo-controlled trial using 200 mg/week nandrolone decanoate for 8 weeks in 16 experienced male bodybuilders. Body mass increased by about 2.2 kg and fat-free mass by about 2.6 kg, with gains concentrated in the trunk and legs and little change in fat mass. Regional DEXA suggested the increases were meaningful for muscle. A 2026 meta-analysis of randomized trials reached a similar conclusion: nandrolone produces modest increases in lean soft tissue but does not consistently improve hand grip strength, knee extension strength, or bone density outcomes in the studied populations.

Real-World Use and Stacking
In practice, nandrolone is rarely run solo. Survey and cohort data show it is among the most common agents in amateur and competitive bodybuilding, typically stacked with testosterone, trenbolone, orals, HGH, and fat-loss drugs. Median cycle lengths around 13 weeks and high average AAS-equivalent doses (near 900 mg/week in one cohort) are common. This polypharmacy and the use of supraphysiologic doses make isolating nandrolone-specific effects difficult. Many users source from the illicit market where labeling accuracy is poor.
Harms and Safety Profile
The harm signals are consistent across reviews. Endocrine issues (hormonal disorders, infertility, gynecomastia), cardiovascular changes (blood pressure, lipids), and reproductive toxicity appear repeatedly. In bodybuilders using nandrolone, lower sperm concentration, motility, testosterone, and FSH have been documented. Long-term cardiac concerns exist, especially when combined with high-dose GH. Real-world risks are amplified by stacking and product quality issues (studies show a high percentage of seized nandrolone samples underdosed relative to label).

Detection and Supply Reality
In doping contexts the main urine targets are 19-norandrosterone (19-NA) and 19-noretiocholanolone (19-NE). Some preparations use isotope signatures that complicate testing. In seized products, a high percentage of nandrolone samples have been found underdosed. In one cohort only 47% of sampled AAS matched the label.
